This is your registration form for Prepared Childbirth Education at Birth
and Beyond. Please make your check payable to Lizabeth Baker-Wade,
RN only. A $50.00 non-refundable fee will be retained. Full refunds
are given to mothers placed on complete bed rest or deliver pre-term.
A full 24-hour notice is required for refunds.
Classes are held for seven days after confirmation by phone
or e-mail. After seven days please reconfirm availability.
If you do not hear from Birth And Beyond, your regsitration has been received and you are confirmed for your class date. Please make a copy of the registration form and keep it for your records.
